To protect the safety and welfare of children, Corvallis School District conducts criminal history background checks on all volunteers. Your information will be submitted to Criminal Information Services INC (CRIS). Confidentiality is maintained and each background check provides a full history. This means that it is important that you answer 'yes' to anything that has occurred, even if it was decades ago. A 'no' answer to an item that shows up on a background check results in an automatic denial. Please be honest in your response and if needed, write a brief explanation in the box provided.

I agree to volunteer for Corvallis School District 509J and agree to the following:

  • I understand that I have no medical coverage as a volunteer if I am hurt or injured.
  • I understand that as a volunteer, I am not covered by the district’s worker’s compensation program.
  • If I am unable to fulfill this agreement, I will notify the district at least 24 hours in advance.
  • I understand I may learn confidential information; I agree to protect the privacy of staff and students.

A check of the applicant's criminal history will be made by the Corvallis School District to verify the responses to the preceding questions.

I understand that any omission, falsely answered statements made by me on this background application shall be grounds for denial to volunteer in the Corvallis School District. Furthermore, I hereby grant to the Corvallis School District permission to check civil or criminal records to verify any statement made on this form. Regardless of whether the applicant grants consent, the Corvallis School District will conduct a criminal offender record check of applicants for all prospective district employees, volunteers, and any other individual working with or around children.
